Shea Medical Center Awarded Third Place in 2015 Quality and Safety Fair

The 2015 UPMC Presbyterian-Shadyside Quality and Safety Fair was held in September. Shea Medical Center, in collaboration with the Department of Pharmacy, was awarded 3rd place for their project titled, “Implementation of a Collaborative Care Management Team to Improve Patient Care.” The goal of this project was to implement a care management team (CMT) to target at-risk patients with uncontrolled diabetes or hypertension. Team members included a pharmacist, social worker, diabetic educator, health plan case manager, and nurses. The CMT collaboratively worked with the patient and their primary care physician to address individual barriers to care and improve disease-state control. The team routinely met to discuss patient care issues and work one-on-one with patients to address individual needs. Based on successful initial results, process improvements were initiated to further increase team impact. The CMT continues to regularly meet to discuss patients. In addition, patients are identified prior to physician appointment so that more proactive interventions can be made.
